Community Health Improvement Grants

The CH I Health Community Health Improvement Grants Program is funded by contributions from our member hospitals. In awarding community grants, CHI Health actively partners with non-profit organizations working to improve health and quality of life in the communities we serve.

Grant funds are used to deliver direct services, strengthen care coordination and improve the health and well-being of underserved populations. The program is one way CHI Health puts its mission and values into practice by investing in programs and partnerships addressing priority community health needs (identified in the most recent community health needs assessment).
