Prescription Discount Card

Eligibility Requirements:

You may be eligible for the CHI Health Prescription Discount Card or the CHI Health Premium Prescription Discount Card if:

1. You do not have prescription insurance coverage through a state or federal healthcare program including but not limited to Medicare part D, Medicaid, Medigap, Veterans Affairs (VA), or Department of Defense (DOD) or TriCare. 
2. You are not utilizing any commercial prescription insurance plan in addition to the CHI Health Prescription Discount Card or the CHI Health Premium Prescription Discount Card for coverage of the prescription.
3. You are 18 years of age or older.
4. You are a resident of the United States or Puerto Rico.

Terms of Use:

1. The CHI Health Prescription Discount Card or the CHI Health Premium Prescription Discount Card is ONLY valid at participating CHI Health Pharmacies. 
2. Patients, pharmacists and prescribers cannot seek reimbursement from health insurance or any third party for any part of the benefit received by the patient for this offer.
3. Acceptance of this offer confirms that this offer is consistent with your insurance and that you will report the value received as may be required by your insurance provider.
4. This offer is void where restricted or prohibited by law.
5. This offer may not be combined with any other rebate/coupon, free trial, or similar offer.
6. This offer is non-transferrable. 
7. This card may not be sold, purchased, traded or counterfeited.  Any reproductions of this card are void.
8. CHI Health reserves the right to rescind, revoke, or amend this offer at any time without notice.
9. This offer is not conditioned on any past, present, or future purchases including refills.
10. No membership fees.
11. CHI Health Prescription Discount Card or the CHI Health Premium Prescription Discount Card are not health insurance.
