Primary Patient Service Centers
- CHI Health patients should have their demographic and insurance information up to date in EPIC. Lab tests should be ordered in EPIC and available to be released from EPIC.
- Non-CHI providers should send their patients with a completed laboratory requisition, copy of demographics and insurance cards.
- Please include alpha numeric diagnosis codes or patients may not be drawn until one is provided
- Medicare patients should have a signed Advanced Beneficiary Notice (ABN) when appropriate (refer to our NCD/LCD page for more information).

You may visit one of the Hospital Diagnostic Centers to have your lab work drawn, or to drop off a specimen.
