Patients:

To become a new patient please request a referral be sent from your primary care physician or physician who has medical records pertaining to your endocrine diagnosis. Once we have received the referral we will call you to schedule your initial visit. When you come to our office for the first time as a new patient, we'll ask you to complete some initial forms, which are listed on this page. We are needing documents #1 through #3 completed.

To make sure there are no delays in care during your first visit, please arrive 30 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.

Please bring the following items to your visit:

  • Your insurance card
  • Valid photo ID
  • List of current medications
  • Office co-pay
  • Completed new patient forms
  • For diabetic patients: Bring blood glucose meter and/or blood sugar log
