At CHI Health, we are committed to improving the health of people at risk for diabetes or those living with diabetes.
In partnership with your physician, our diabetes educators provide a variety of outpatient services at most CHI Health locations.
A physician referral is required for individual instruction, group classes, nutrition therapy and gestational education. We recommend patients call insurance to check on their benefits for diabetes education prior to the first visit. Provide the codes below to your insurance company to learn what opportunities you have for education. If your specific plan does not cover these visits, we have cash-pay options available that you may qualify for.
Diabetes Education Billing Codes:
A referral is needed to see our Diabetes Education team. With a referral, you can call to schedule, (402) 717-9115, or chat our scheduling team. Chat agents available Monday - Friday, 8 a.m to 5 p.m.
When you get together with others who have Type 1 diabetes, you can encourage each other, share ideas, talk about your experiences, and find new ways to cope with your condition. This year’s groups will be virtual.

People living with type 2 diabetes are invited to learn self-care strategies and share real life experiences at our support groups. Currently supports groups are being held virtually. Whether you need a jump start or motivation to keep improving, we are here to help!
