Transitional Care Team

Transitional Care offers increased therapy services geared specifically to your recovery. You will meet with a local hospitalist or a telehospitalist weekly to discuss your progress. With our low patient-to-staff ratio, you will receive more personalized care and support from expert nursing, therapy and medical staff.

Our multidisciplinary approach means care is individualized to your needs. An entire team is focused on best assessing your progress and providing the most complete care.
