Community Health Needs Assessment

Creating healthy communities is central to CHI Health St. Mary’s mission, vision and tradition. CHI Health St. Mary’s follows a rigorous process to assess and address unmet community health needs. A Community Health Needs Assessment (CHNA) is a process whereby health related data and community input are gathered to identify and prioritize the most important health needs of the community.

Following the CHNA process the hospital develops an Implementation Strategy Plan (ISP) to leverage hospital and community assets to act upon the unmet community health needs.

CHI Health St. Mary’s uses the CHNA process to guide healthier communities work, and community benefit investments.
