Patients must be referred to psychiatric home care by a physician or other healthcare worker.

The program potentially enables earlier discharge from psychiatric inpatient stays and allows for the continuation of care in the community by teaching a patient to cope with stress through effective communication skills and by emphasizing positive coping strategies.
Assistance is also provided with medication, diet and in helping the patient monitor symptoms to maintain wellness.
Working closely with the primary care physician, each patient’s care is coordinated through our psychiatric team, including nurses, medical social workers, physical, occupational and speech therapists, home health aides, and secondary care providers involved with the patient.
A comprehensive evaluation of the patient is conducted in the home and in community environments to help the patient increase independent living skills and move toward mental wellness.
