Community Health Improvement Grants

The CHI Health Community Health Improvement Grants Program is funded by contributions from our member hospitals. In awarding community grants, CHI Health actively partners with non-profit organizations working to improve health and quality of life in the communities we serve.

Grant funds are used to deliver direct services, strengthen care coordination and improve the health and well-being of underserved populations. The program is one way CHI Health puts its mission and values into practice by investing in programs and partnerships addressing priority community health needs.

Funding Criteria

The Community Health Improvement Grants Program uses funding criteria to ensure that its charitable resources address health needs in ways that demonstrate accountability for impact. Grant applicants should ensure that their proposed projects: 

  • Address one or more prioritized significant needs in the local hospital’s Community Health Needs Assessment (CHNA) and Implementation Strategy.
  • Identify a specific community and primary population the program or project will serve.
  • Include two or more collaborating organizations with distinct, complementary and substantive project roles.
  • Identify specific and measurable project outcomes.

Timeline

July 17 – September 8, 2023: Grants application period is open. Applicant organization can apply online.

November 20, 2023: Applications are notified of funding decisions.

December 2023: Grant agreements are signed, followed by grant award payments.

January 1, 2024: Grant project period begins.

July 31, 2024: Mid-year project reports due.

December 31, 2024: Grant project period ends.

February 15, 2025: Final project reports due. 

Eligibility and Parameters

  • Lead applicant must be a 501(c)(3) non-profit organization.
  • Grant awards range from $2,000 up to $50,000.
  • Mid-year and final written reports are required.

Application Process

  1. Review your local CHI Health hospital's priority community health needs (refer to the Community Health Needs Assessment and Hospital Implementation Strategy).
  2. Review the Community Health Improvement Grant application and apply online.
  3. Optional - Contact the Healthy Communities team to discuss your project proposal.
