Honor or Memorial Gifts

Any gift can be made in memory or in honor of a special person or event. In the Tribute section please indicate the name of the person you wish to honor and provide a mailing address as indicated. You can also mail in your gift to the address below. Include in a short note or in the memo portion of your check (payable to CHI Health Foundation), the name of the person you wish to honor or memorialize, and provide a mailing address. We will be honored to send a notification of your generous remembrance to the individual(s) honored, or the family of the individual(s) memorialized. Your gift amount is not disclosed.

"The family requests that memorial gifts be forwarded to:"

"Gifts in memory of 'Mr. Jones' may be made to the CHI Health Rehabilitation Care at CHI Health Immanuel."
