Medicare Annual Wellness Visit

As a patient on Medicare you are eligible for a free Annual Wellness Visit (AWV).

Here at CHI Health, we get overwhelmingly positive reviews from the patients who have already had their Annual Wellness Visit. They love the personalized attention they get from the nurse and they feel very taken care of. We want to make sure you are well taken care of for the years ahead.
