Antoinette Hardy-Waller, a representative of Catholic Health Initiatives, has served as a Director on the CHI Health Board since 2010. In addition, she is a member of CHI Health’s Patient Safety and Quality, and Planning Board Committees.
Ms. Waller is Founder and Past President/Chief Executive Officer of CareCollaborative Home Health Services, Inc., a licensed and Medicare certified home health agency.
As President/CEO of her first agency, Hardy Associates Home Health Services, Inc., she grew it to become one of the largest, most progressive, urban-based home health agencies in Chicago, Illinois. Ms. Waller has consulted nationally with major health care organizations to assist with the linkage of acute and post acute care services through the implementation of effective integration strategies.
Ms. Waller has served on both the national and local home care boards. She was recently appointed to the Board of Trustees for Catholic Health Initiatives, the second largest Catholic health system in the country. Other board seats include the National Association for Health Services Executives, former President Chicago Chapter, Co-Chair National CEO/Senior Executive Conference, the American Hospital Association Center for Healthcare Governance, the Illinois Department of Public Health Home Health Advisory Board and the Have A Heart for Sickle Cell Anemia Foundation.
She recently was inducted into Who's Who in Black Chicago. She received her Bachelor's of Science Degree in Nursing from the University Of Illinois College Of Nursing and holds a Masters in Jurisprudence from Loyola University School of Law.
