Community Health Needs Assessments (FY-13-15)
The mission of Alegent Creighton Health calls us to collaborate with others to improve the health of the communities we serve. Creating healthy communities is central to our mission, vision and tradition. The Patient Protection and Accountable Care Act adds new requirements upon not-for-profit hospitals to collaborate with the community to identify health needs and develop strategies to address the needs identified.
Alegent Creighton Health partners with local health departments and community representatives to conduct a “Community Health Needs Assessment” (CHNA) every 3 years. A CHNA is a process whereby the community gathers and evaluates health related data and seeks community input to identify and prioritize the health needs of the particular community. A community action plan is then developed to leverage community assets and act upon the unmet community needs. Leadership from Alegent Creighton Health hospitals led or participated in these local processes. We use the CHNA process to identify participation in community coalitions and to guide our Community Benefit investments.
