2015 Scholarship Program

We are pleased to announce that CHI Health will once again provide 12 - $1,500 non-renewable scholarships to Nebraska and/or Iowa high school seniors graduating May 2015.The goal of this program is to provide scholarship assistance to eligible students preparing for a career in the healthcare industry.


Eligibility:

1)  NE or IA high school seniors graduating May 2015 with intent to pursue a post secondary degree in a health care field (excluding Dentistry and Veterinary Science fields).

2)  Applicants must have applied to, and plan to enroll in an accredited college/university.

3)  Applicants must have and maintain a minimum cumulative GPA of 3.0 or higher on a 4.0 non-weighted scale.

Scholarship Application: 

The application is a writable PDF and needs to be completed on a computer. The application deadline is Monday, February 9, 2015.

 

Applicant Appraisal Form:

The student is to print and give the Applicant Appraisal Form to one high school administrator, high school guidance counselor or teacher to complete. (No family members can complete this form.

 

High School Transcript

An official high school transcript with seal is required. High schools may mail them directly or transcripts that are sealed in an envelope with signature across the flap can be included in the student’s application packet.
