The goal of physical medicine and rehabilitation is to restore the injured or disabled person to the highest level of function possible. With all the choices available, it is difficult to know which rehabilitation center can best meet the patient's needs and help improve the quality of life for the long term. The following are questions to ask and factors to consider when selecting a rehabilitation center:

Programs

Does the center provide rehabilitation exclusively?

When a program is focused exclusively on providing rehabilitation services, they concentrate all of their efforts and resources to the needs of individuals with physical disabilities. Even the facility itself should be equipped to provide access to the disabled.

At CHI Health Rehabiliation Care, our programs and staff are devoted exclusively to providing physical rehabilitation services. The design of our facility is focused on those with physical disabilities, from automatic doors and wide hallways to wheelchair accessible drinking fountains and restrooms. It is a care center within the Immanuel Medical Center campus, with access to the full medical services of the medical center, 24 hours a day.Our broad range of services enable us to treat patients of all ages with physical disabilities resulting from:

  • strokes
  • spinal cord injuries
  • neuromuscular and skeletal disorders
  • cerebral palsy
  • muscular dystrophy
  • multiple sclerosis
  • brain trauma
  • amputations
  • peripheral nerve injuries
  • arthritis
  • burns

Our new inpatient unit consists of 35 private patient rooms adjacent to a newly renovated and upgraded inpatient therapy gym. Each patient room has private bathroom including a shower. A dining area and common spaces are included. Patients, family members and friends can find mutual support and encouragement from each other.

At CHI Health Rehabiliation Care at Immanuel, we offer the region's most comprehensive range of physical rehabilitation services, including:

Are educational services provided?

Rehabilitation is a process of learning physically and mentally. To ensure long-term success, education should be an integral part of the rehabilitative therapy program. The education process begins from the first day a patient enters our facility. By educating both the patient and family, we make the transition from CHI Health Rehabiliation Care to home easier. Education ensures the continuity of the recovery process.

Is the family involved in therapy?

The family of a patient plays an important role in the process of rehabilitation. They provide emotional and other support to the patient. And, they are often charged with facilitating continued rehabilitation after the patient is dismissed from an inpatient care facility. The rehabilitation center should involve family members in the care plan and provide support services that help them cope with the situation.

At CHI Health Rehabiliation Care family members are informed of progress at all stages. Family members are encouraged to be actively involved in planning and delivering the patient's physical therapy. Staff members are available to provide support to family members. We offer free parking and help to arrange overnight accommodations.

Are services easily accessible?

While the type of care available is important, so too, is the accessibility of services. It may help to select a center that provides both inpatient and outpatient services, or one that has multiple locations that are convenient. CHI Health Rehabiliation Care is located on the Immanuel Medical Center campus, which off offers inpatient, outpatient and subacute services. We also have several sites located throughout the Omaha area for outpatient therapy services.

Is customized equipment available?

Even after therapy, patients may need assistive devices to help them function independently outside the rehabilitation center. The rehabilitation center should offer assistance in fitting these devices and their use. Equipment within the center that simulates real-life situations, such as a trip to the store or bank, can help the patient return to community life.

At CHI Health Rehabilitation Center, we provide assistance with the acquisition and use of assistive devices such as walkers, wheelchairs, prosthetic devices, etc. We simulate real-life situations to better prepare the patient to function independently at home or work. We make recommendations on home, work and car modifications and equipment to improve the patient's ability to function effectively.

Is follow-up care available?

Even after the acute physical rehabilitation process is complete, there are other steps that must be taken to integrate the patient back into the community. The rehabilitation center should provide some form of follow-up care such as support groups, job training, vocational counseling, outpatient therapy and more. We coordinate follow-up services with other agencies in the patient's hometown. CHI Health Rehabiliation Care patients have access to a wide range of follow-up care including: outpatient therapy, vocational counseling, job training, educational services and counseling.

Is emergency medical care available?

Medical emergencies can happen anywhere. That is why it is important that the rehabilitation center have access to emergency medical care. And, its personnel should have training that lets them know how to respond in the case of an emergency. At CHI Health Rehabiliation Care . . . Our staff is fully trained to respond to a medical emergency. If additional care is required, we are located adjacent to CHI Health Immanuel for immediate access to emergency medical personnel and services.

Does it have a specialized, experienced Staff?

Since the staff at the rehabilitation center will be providing all the care, it is important to look at their educational backgrounds, training and experience. It is also important to find out about their philosophy of care. That means making sure they are not just concerned about the physical function of the patient, but also other areas of recovery. Immanuel Rehabilitation Center uses a team approach to recovery. Our complete staff of licensed and certified professionals represents such areas as medicine, therapy, counseling, psychology and nursing. We have the region's largest staff of board-certified physiatrists, physicians specializing in physical medicine and rehabilitation. Our philosophy of care encompasses the recovery of the body, mind and spirit.

Is the Program effective?
Is there enough staff to provide individualized attention?

The rehabilitation center should have a sufficient number of staff members to provide individualized therapeutic care. A large staff also enables the patient to work with the same team of professionals for a level of consistency and comaraderie that comes from working together over time. This high level of personal care leads to achieving optimal results.

At CHI Health Rehabilitation Center at Immanuel, our experienced staff members provide individualized attention for each patient. Patients work with a dedicated team of professionals to build a level of trust and comfort that leads to more positive results. On average, acute care patients receive a minimum of three to five hours of structured therapy per day. This varies with the seriousness of the patient's condition.

  • Our ultimate goal is to help the patient return home or to work.
  • Our professional rehabilitation staff, together with the patient and family, develop a plan with specific measurable goals for each individual. All therapy and progress is well documented.
  • Regular communication helps to gauge progress, determine necessary changes and prepare patients for discharge.
  • We exceed the national average in number of patients who are able to return home following therapy.
